We are increasingly enabling patients to request repeat prescriptions and book appointments on line and, more recently have been offering access to your patient record.
We are able to offer patients access to coded data but this requires additional considerations as outlined in the Accessing GP Records Online information leaflet below.

You will be asked that you have read and understood this leaflet before consenting and applying to access your records online and the practice will also need to verify your identity. If you would like access to these services please print off and complete the registration form below after reading the above information leaflet.
Patients who already have access to the online booking of appointments and requesting repeat prescriptions will also need to complete the Patient Online Registration form to request access to their records.
